Interruption Insurance Claims

If you had to return home early from your trip due to a sickness, injury or another unexpected covered situation that occurred during your trip, you will need to submit a Trip Interruption Insurance claim.


How to File a Trip Interruption Insurance Claim

Step 1.

Print and fill out your portion of the Claim and Authorization Form pdf.

Note: This form is provided in Adobe PDF format. A recent version of Adobe Reader or Adobe Acrobat may be required to open, read and print the form on your system. You can install the latest version of Adobe Reader by clicking here .

Step 2.

If the interruption is due to an injury or sickness (sustained by you, an immediate family member or your traveling companion), have the attending physician who provided treatment complete the appropriate section on the Claim and Authorization Form.

Step 3.

Gather together the following supporting documents as applicable, which you will need to send with your completed Claim and Authorization Form:

  • An itemized copy of your invoice that provides a breakdown of the cost of your trip (airfare, hotel, taxes, service fees, etc.) In addition, please provide one of the following as proof of payment:
    • Copy of credit card statement
    • Copy of canceled check
    • Copy of debit card slip
    • Copy of official receipt issued by your travel agent for a cash payment
  • Original unused airline ticket(s) along with the new return tickets purchased. Or, the original receipts for the change fees paid.
  • If the trip interruption is for non-medical reasons, please provide substantiation:
    • Report from the airline/cruise line
    • Death certificate
    • Copy of subpoena for court/jury duty
    • Letter from employer
  • If you incurred additional expenses as a result of one of the insured risks, please submit your original itemized receipts for additional commercial accommodations and meals, essential telephone calls and taxi fares.

Step 4.

Once completed, sign the Claim and Authorization Form, make a copy for your records, and then mail the original signed form, along with all applicable supporting documents noted above, to our Claims office:
