Trip Interruption/Delay

Trip Cancellation Claim:

Important: As outlined in your policy, when a cause of cancellation occurs before the date of departure, you must cancel your trip with the Travel Agent or the carrier immediately, but no later than the business day following the cause of cancellation and advise us at the same time. To pursue your claim for losses due to a cancelled trip:

  1. Please print and fully complete the CLAIM AND AUTHORIZATION FORM. Once completed, sign a copy of the form, and then mail the form, along with all supporting documents noted below, to the Claims office.

  2. If the cancellation is due to an injury or sickness of yourself, an immediate family member or your traveling companion, you must have the Medical Certificate fully completed by the medical doctor who treated the person whose sickness or injury has caused the cancellation of the trip.
  3. Submit an itemized copy of your invoice, providing a breakdown of the cost of your trip - airfare, hotel, taxes, service fees, etc.In addition, please provide one of the following as proof of payment:
    1. copy of credit card statement
    2. copy of cancelled check
    3. copy of debit card slip
    4. copy of official receipt issued by your travel agent for a cash payment
  4. Attach your original unused airline ticket(s) and any other original travel documents. If you have received a refund from your travel agent or the travel supplier, please provide a copy of the statement or refund.
  5. If canceling for non-medical reasons, please provide substantiation:
    1. copy of subpoena for court/jury duty
    2. letter from employer
    3. death certificate
    4. report from the airline/cruiseline/transporation authority
  6. Copy of Terms and Conditions from the travel supplier indicating the cancellation penalty.

Trip Interruption/Delay Claim:

Situations can arise that cause you to unexpectedly return home early or delay your originally scheduled return. When you contacted Assured Assistance Inc. to advise of your situation, you were advised that we would require a completed claim form and supporting documentation.

  1. Please print and fully complete the CLAIM AND AUTHORIZATION FORM. Once completed, sign a copy of the form, and then mail the form, along with all supporting documents noted below, to the Claims office.

  2. If the interruption is due to an injury or sickness, you must have the Medical Certificate fully completed by the medical doctor who treated the person whose sickness or injury has caused interruption of your trip.
  3. Submit an itemized copy of your invoice, providing a breakdown of the cost of your trip - airfare, hotel, taxes, service fees, etc. In addition, please provide one of the following as proof of payment
    1. copy of credit card statement
    2. copy of cancelled check
    3. copy of debit card slip
    4. copy of official receipt issued by your travel agent for a cash payment
  4. Attach original unused airline ticket(s) along with the new return tickets purchased. Or the original receipts for change fees paid.
  5. If the cause of the interruption is a non-medical reason, please provide substantiation:
    1. report from airline/cruise line
    2. letter from the employer
    3. death certificate
    4. Copy of subpoena for court/jury duty
  6. If you incurred additional expenses as a result of one of the insured risks, please submit your original itemized receipts for additional commercial accommodations and meals, essential telephone calls and taxi fares.
